That one bugs me, surely it should be two grand dads as the hyphenated name usually is a mix of the mother's maiden name and the father's surname.
No, usually, as in my case, it is a combination of my biological (deceased) father and my adoptive father's surnames. your suggestion is usually the exception to the rule.

With such low adoption rates I doubt it. Most of the people I know with double-barrelled names are a combination of mother's maiden name and father's name. I agree it could be a combination of biological father's & step-father's names.
